  • Auto-Refill is an additional service option you may choose while enrolling in Akorn EyeRx Direct®. Patients with commercial (private) insurance who elect Auto-Refill will pay as low as $25 for each bottle or carton of medication — an additional savings of $20 for every month of therapy shipped to your door.
  • All patients with a valid prescription may sign-up for the Auto-Refill option and receive the convenience of automatic monthly refills. However, only patients using commercial (private) insurance to pay for their medication will be able to take advantage of the additional savings.*

What types of insurance does the Akorn EyeRx Direct® program accept?
  • The Akorn EyeRx Direct program accepts patients with commercial (private) drug insurance, Medicare Part D, Medicaid, as well as other state and federally funded medical assistance programs, such as VA/Military.*
  • However, depending on your circumstances and potential restrictions that may be present within your insurance plan your orders may not be processed utilizing your insurance plan. If your order is not processed through your insurance plan you will instead be charged a fixed direct-to-patient price of $45 per bottle or carton.

I have private commercial insurance. What will I pay with Akorn EyeRx Direct®?
  • You will pay no more than $45 per bottle or carton of therapy, and may pay as little as $25.*
  • If you have commercial (private) insurance and also enroll in the Auto-Refill option then your payment for medication will be lowered to as little as $25 — an additional savings of $20 every month.
I have Medicare Part D. Can I enroll and what will I pay for my prescription?
  • All patients with Medicare Part D prescription drug coverage insurance are eligible to enroll and participate in the Akorn EyeRx Direct® program.
  • However, most prescriptions will not be processed through Medicare Part D and you will instead pay the direct-to-patient price of $45 per bottle or carton of medication.*
What are the payment options for using Akorn EyeRx Direct®? Can I use a debit card?
  • EyeRx Direct accepts credit and debit cards from VISA®, Mastercard®, American Express® and Discover®. Akorn EyeRx Direct also accepts payment from Flexible Spending Accounts (FSAs) and Health Savings Accounts (HSAs).
  • PayPal®, cash, personal checks, and money orders are not accepted.

How many days or months worth of medication may I order at a time?
  • The EyeRx Direct program allows you to order more than one 30-day* (one month) supply of your medication at a time.
  • However, this is only possible if the prescription written for you by your eye doctor authorizes more than one month of therapy to be dispensed to you at a time.
  • Also, in those situations where your insurance is paying for all or a portion of the cost of your medication, your insurance carrier must also allow more than one month of therapy to be dispensed to you at one time.

How will my medication arrive?
  • Your medication will be sent directly to you at no extra charge by our specialty pharmacy partner Eagle Pharmacy and will arrive to you from either the US Postal Service or UPS, depending on which medication you ordered.
  • It will be sent to the address you specify and will be delivered without the need for a signature, unless you specify otherwise.
  • The packaging is discreetly marked so its contents are not evident.
It helps to know when packages are coming. Is there a way to track the status of my order?
  • Yes! You will always receive an email letting you know when the order was shipped to you. Refills will arrive at your door within 3 to 5 days of ordering.
  • Also, once you have enrolled in Akorn EyeRx Direct®, you may log on to the Eagle Pharmacy website at any time to check the status of your order at www.EaglePharmacy.com

ZIOPTAN® Indication
ZIOPTAN® (tafluprost ophthalmic solution) 0.0015% is a prostaglandin analog indicated for reducing elevated intraocular pressure in patients with open‑angle glaucoma or ocular hypertension.

ZIOPTAN® Important Safety Information
ZIOPTAN® has been reported to cause changes to pigmented tissues. The most frequently reported changes have been increased pigmentation of the iris, periorbital tissue (eyelid), and eyelashes. Pigmentation is expected to increase as long as ZIOPTAN® is administered. After discontinuation of ZIOPTAN®, pigmentation of the iris is likely to be permanent, while pigmentation of the periorbital tissue and eyelash changes have been reported to be reversible in some patients. Patients who receive treatment should be informed of the possibility of increased pigmentation. The long-term effects of increased pigmentation are not known. Iris color change may not be noticeable for several months to years. While treatment with ZIOPTAN® can be continued in patients who develop noticeably increased iris pigmentation, these patients should be examined regularly.

ZIOPTAN® may gradually change eyelashes and vellus hair in the treated eye. These changes include increased length, color, thickness, shape, and number of lashes. Eyelash changes are usually reversible on discontinuation of treatment.

ZIOPTAN® should be used with caution in patients with active intraocular inflammation (e.g., iritis/uveitis) because the inflammation may be exacerbated. Macular edema, including cystoid macular edema, has been reported during treatment with prostaglandin F2α analogs. ZIOPTAN® should be used with caution in aphakic patients, in pseudophakic patients with a torn posterior lens capsule, or in patients with known risk factors for macular edema.

In clinical trials of patients receiving either preservative-containing or preservative-free ZIOPTAN®, the most common pooled adverse reaction observed was conjunctival hyperemia, which was reported in a range of 4% to 20% of patients.

COSOPT® PF Indication
COSOPT® PF is a carbonic anhydrase inhibitor with a beta-adrenergic receptor blocking agent indicated for the reduction of elevated intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension who are insufficiently responsive to beta-blockers.

COSOPT® PF Important Safety Information
COSOPT® PF is contraindicated in patients with bronchial asthma, a history of bronchial asthma, or severe chronic obstructive pulmonary disease, sinus bradycardia, second or third degree atrioventricular block, overt cardiac failure, and cardiogenic shock or hypersensitivity to any component of this product.

COSOPT® PF contains timolol maleate, a beta-adrenergic blocking agent; and although administered topically, is absorbed systemically. Therefore, the same types of adverse reactions that are attributable to systemic administration of beta-adrenergic blocking agents may occur with topical administration. For example, severe respiratory reactions, including death due to bronchospasm in patients with asthma, and rarely death in association with cardiac failure, have been reported following systemic or ophthalmic administration of timolol maleate.

BETIMOL® Indication
Betimol® is indicated in the treatment of elevated intraocular pressure in patients with ocular hypertension or open‑angle glaucoma.

BETIMOL® Important Safety Information
Betimol® is contraindicated in patients with overt heart failure, cardiogenic shock, sinus bradycardia, second- or third-degree atrioventricular block, bronchial asthma or history of bronchial asthma, or severe chronic obstructive pulmonary disease, or hypersensitivity to any component of this product.

The most frequently reported ocular event in clinical trials was burning/stinging on instillation and was comparable between Betimol and timolol maleate (approximately one in eight patients).

AZASITE® Indication

AzaSite is indicated for the treatment of bacterial conjunctivitis caused by susceptible isolates of the following microorganisms: CDC coryneform group G*, Haemophilus influenzae, Staphylococcus aureus, Streptococcus mitis group, Streptococcus pneumoniae.

*Efficacy for this organism was studied in fewer than 10 infections.

AZASITE® Important Safety Information

AzaSite is contraindicated in patients with hypersensitivity to any component of this product.

AzaSite is NOT FOR INJECTION. AzaSite is for topical ophthalmic use only and should not be administered systemically, injected subconjunctivally, or introduced directly into the anterior chamber of the eye.

In patients receiving systemically administered azithromycin, serious allergic reactions, including angioedema, anaphylaxis, and dermatologic reactions, including Stevens-Johnson syndrome and toxic epidermal necrolysis, have been reported rarely. Although rare, fatalities have been reported.

As with other anti-infectives, prolonged use may result in overgrowth of nonsusceptible organisms, including fungi. If super-infection occurs, discontinue use and institute alternative therapy.

Patients should be advised not to wear contact lenses if they have signs or symptoms of bacterial conjunctivitis.

The most frequently reported ocular adverse reaction reported in clinical trials was eye irritation, which occurred in 1% to 2% of patients. Other adverse reactions associated with the use of AzaSite were reported in less than 1% of patients and included ocular reactions (blurred vision, burning, stinging and irritation upon instillation, contact dermatitis, corneal erosion, dry eye, eye pain, itching, ocular discharge, punctate keratitis, visual acuity reduction) and nonocular reactions (dysgeusia, facial swelling, hives, nasal congestion, periocular swelling, rash, sinusitis, urticaria).

There are no adequate and well-controlled studies in pregnant women. Azithromycin should be used during pregnancy only if clearly needed.

It is not known whether azithromycin is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised when azithromycin is administered to a nursing woman.

Safety and effectiveness of AzaSite solution in pediatric patients below 1 year of age have not been established.

No overall differences in safety or effectiveness have been observed between elderly and younger patients.

The recommended dosage regimen for the treatment of bacterial conjunctivitis is: Instill 1 drop in the affected eye(s) twice daily, 8 to 12 hours apart, for the first 2 days and then instill 1 drop in the affected eye(s) once daily for the next 5 days.

Store unopened bottle under refrigeration at 2°C to 8°C (36°F–46°F). Once the bottle is opened, store at 2°C to 25°C (36°F–77°F) for up to 14 days. Discard after the 14 days.

  • All patients over the age of 18 and residing in the continental United States with a valid prescription for Zioptan®, Cosopt® PF, Betimol® or AzaSite® are eligible to participate in the Akorn EyeRx Direct Program.
  • Patients with commercial insurance covering the products in this program will pay their usual and customary co-pay amount for a bottle or carton of product, not to exceed $45.
  • Patients with commercial insurance covering Zioptan®, Cosopt® PF, or Betimol® who also elect to enroll in the Auto-Refill option will receive additional savings of up to $20 so they pay no more than $25 per month of product ordered.  Patients with commercial insurance covering AzaSite® are not eligible to receive the additional discount for the Auto-Refill option.
  • For patients with commercial insurance plans that either restrict coverage of the products in this program or that require an out-of-pocket payment greater than $45 for a bottle or carton, orders will not be processed through the commercial insurance. Instead, these patients will pay a direct-to-patient Akorn EyeRx Direct price of $45 for each bottle or carton of therapy ordered.
  • For patients who do not use commercial insurance and instead pay the direct-to-patient Akorn EyeRx Direct price of $45 per bottle or carton of therapy, the additional discount for the Auto-Refill option is not available.
  • Patients with Medicare Part D that 1) do not have insurance restrictions, and 2) have a co-pay amount of $45 or less for a bottle or carton of products in this program, will pay their usual and customary co-pay amount.
  • For patients with a Medicare Part D insurance plan that has restrictions or a required co-pay amount of more than $45 for a bottle or carton of product, orders will not be processed through Medicare Part D. Instead, these patients will pay a direct-to-patient Akorn EyeRx Direct price of $45 for each bottle or carton of therapy ordered. Medicare Part D orders processed at the direct-to-patient Akorn EyeRx Direct price cannot be applied to Medicare Part D true out of pocket (TrOOP) costs.
  • Patients with Medicaid or other state or federally funded medical assistance programs (VA / Military, etc.) that have restrictions or require a co-pay greater than $45  per bottle or carton of prescribed product will not be processed using the assistance program(s). Instead these patients, as well as those patients without insurance, will pay a direct-to-patient EyeRx Direct price of $45 for each bottle or carton of product therapy ordered.
